Clinical Case Manager - (National)

Our client provides a range of services and programs to support veterans and their families. Currently they have multiple openings in most of the major capital cities for experienced Clinical Case Managers, providing high-quality mental health assessments and clinical counselling services for veterans and their families.

The successful candidates will demonstrate previous experience managing a case load of clients. You will be responsible for providing an escalation point for clients at risk of harm to self or others, triaging, assessment and providing specialised case management via telephone and contributing to record keeping and maintaining high level and quality documentation.

In order to be successful with your application, you will have/demonstrated the following;

  • Tertiary qualifications in a relevant health field including Nursing, Psychology, OT or Social Work and ideally registered, or eligibly to register with the Australian Health Practitioners Registration Agency (AHPRA) or a social worker accredited with the Australian Association of Social Workers (AASW)
  • Demonstrated experience in complex mental health clinical service delivery, including intake, triage, and risk assessment.
  • Extensive experience working within a mental health or community case setting, including working with people who present with complex mental health conditions
  • Experience working with military or veteran mental health services is desirable but not essential
